1. Objective
This Standard Operating Procedure (SOP) defines the steps and responsibilities for identifying, reporting, tracking, and escalating AI-related incidents in the AI-SDLC environment. An AI-related incident is any anomaly or unexpected event caused or impacted by an AI component that could result in system disruption, data compromise, adverse user impact, or risk to compliance/regulatory standards. The AI-IRB (Artificial Intelligence Institutional Review Board) is a key stakeholder in determining compliance or advanced risk escalations.
2. Scope
This SOP covers the lifecycle of AI incident handling and escalation, including detection, triage, resolution, and the required follow-up in the AI-SDLC environment. It applies to all AI-related services, platforms, or processes within the organization. This includes:
- AI system anomalies detected during testing or production.
- Compliance or ethics concerns about an AI system’s outputs.
- Data or privacy issues caused by AI modules.
3. Applicability
All SDLC teams working with AI components or integrated AI systems must follow the process detailed in this SOP. The main roles include:
- Incident Reporter: Anyone (internal or external) identifying a potential AI incident.
- Incident Manager: Responsible for triage and oversight of incident resolution.
- Development: Handles code or model changes if needed.
- AI-IRB Liaison: Evaluates the severity and compliance implications of the AI incident.
- Operations: Ensures infrastructure or environment changes are managed properly.
- Quality Assurance (QA): Validates or re-tests fixes for impacted AI modules.
- Technical Support (TS): Receives and screens external user/client-reported incidents.
- Security/Compliance: Engaged if there is a potential regulatory or data protection risk.
4. Definitions and Acronyms

| Authorized AI Agent | AI entities or subroutines officially sanctioned by the AI-IRB to perform autonomous or semi-autonomous tasks within the scope of this procedure. |
5. Roles and Responsibilities

6. Procedure Activities
6.1 Incident Detection and Reporting
- Identify AI Incident
- Triggered by system alerts, error logs, user complaints, abnormal model outputs, or recursive self-improvement subroutine anomalies.

- The system or TS routes the ticket to an Incident Manager. That manager is then responsible for oversight.
6.2 Triage and Initial Assessment
- Categorize Severity
- The Incident Manager reviews the details to determine severity (low, medium, high, critical).
- If there’s potential regulatory or ethical risk, the manager notifies the AI-IRB Liaison.
- Prioritize
- The Incident Manager sets priority based on scope of impact:
- Critical: Widespread user impact or severe compliance risk.
- High: Significant performance or output quality issue.
- Medium: Localized or intermittent failures.
- Low: Minor inconvenience or documentation fix.

- Notify Stakeholders
- The Incident Manager sends notifications to relevant stakeholders (e.g., Development, Ops, QA, AI-IRB Liaison if needed).
- AI-IRB Escalation Check
- If the incident involves potential harm, compliance concerns, or major model bias:
- The AI-IRB Liaison reviews the incident details.
- The Liaison decides if formal AI-IRB input is required. If yes, the AI-IRB meets or delegates next steps.

6.3 Investigation and Analysis
- Root-Cause Analysis
- Development or Ops examines logs, data, model outputs, and system conditions to isolate the root cause.
- If the incident is purely software logic or environment-based, Ops or Dev proceed with normal analysis.
- For model drift or bias issues, specialized data scientists or ML engineers get involved.
- Mitigation Approach
- Proposed fix or workaround is documented.
- If the fix involves changes to an AI model, the AI-IRB Liaison must confirm compliance with re-training data or new model parameters.
- Test/Validation Plan
- Quality Assurance drafts a plan to verify the fix.
- If the fix is urgent, a “hot patch” testing approach is used with minimal scope.
- For deeper issues, a more extensive test environment is prepared.
6.4 Fix Implementation
- Implement Fix
- Development modifies code, model parameters, or pipeline configuration.
- The fix is version-controlled and documented, referencing the incident ticket.
- Perform QA Tests
- Quality Assurance runs regression tests to ensure fix addresses the incident without new regressions.
- Testing can happen in a staging environment or specialized “AI test environment.”
- Operations Support
- Ops deploys the fix if QA tests are successful.
- For critical incidents, a direct push to production might be authorized, with standard or emergency push procedures.
6.5 Validation and Closure
- Incident Retest
- The Incident Manager ensures the updated system is retested in the environment where the incident occurred.
- If the issue reoccurs, it re-opens for further analysis.
- Accept or Rework
- If retest passes, the Incident Manager changes the ticket status to “Resolved.”
- If retest fails, the fix is reworked (return to Fix Implementation).
- Escalation to AI-IRB
- If post-fix analysis indicates deeper model changes or data usage changes, a final review from AI-IRB may be required prior to official closure.
- Close Incident
- Once everything is verified, the Incident Manager closes the ticket.
- Technical Support is updated for external communication, if needed.
6.6 Post-Incident Follow-Up
- Document Lessons Learned
- The Incident Manager records root-cause findings and recommended improvements in the Lessons Learned repository.
- Update AI Artifacts
- Model or pipeline changes are integrated into the standard AI lifecycle documentation.
- If any compliance or ethics considerations are discovered, the outcome is flagged for subsequent AI-IRB reviews.
- Continuous Monitoring
- Enhanced monitoring or additional checks may be implemented to detect recurrences early, integrating Exochain Peer Reviews for all automated incident responses.
7. Forms and Records
No unique forms are mandated by this SOP. Existing incident logging and change control forms apply. However, the following references might be used:
- Incident Ticket (e.g., from SQA Manager or centralized tool).
- AI-IRB Escalation note or email template.
- Lesson Learned record or database entry.
8. Exceptions
- Emergency Incidents: If a showstopper (e.g., major compliance risk) is discovered, the fix can be escalated using an emergency push procedure. Documentation is still required but may be completed retroactively.
- Non-AI Incidents: Incidents that do not involve AI or machine learning are managed by standard non-AI incident processes.
