FINAL: Learning from Failure

Scenario

A system your board approved at G-5 has caused documented harm. The post-incident review finds the signal was present in the record at the time of approval — raised, discussed, and dismissed with reasoning that seemed adequate and was not.

Your task

Write the failure analysis and the correction. Name what failed without softening it and without converting it into blame theater. Specify what changes in the gate record — and, following the discipline that governs this curriculum, what is recorded as a dated correction rather than rewritten. State who is told, in what register, including the people harmed. Then specify the structural change that makes the same dismissal harder next time, and what it costs.

What the council weighs

Blamelessness with accountability held together. Accounts that protect the record by burying the failure, or protect the people by burning the record, both score low. The correction must be verifiable by someone who distrusts you.

Submission

1,000–1,500 words. AI assistance is permitted and must be disclosed in your attestation; undisclosed assistance is grounds for revocation. Graded by council against the five rubric dimensions in the assessment specification.
